The Anti-Aging Pill: Dr. Marting Shares Her Advice

Dr. Marting shares her advice on living a long and beautiful life…

You can’t listen to TV, radio, read a magazine, or browse the internet without encountering multiple messages informing us that anti-aging is possible, “If you only take this supplement, or try that regimen”. Miracle IV infusions have become a one of the more recent “fad” treatments that, I hope, people are undergoing only with extreme caution, if at all. All sorts of hormone replacements are touted regularly, all in the hopes of holding off the dreaded effects of aging, if not eliminating them entirely. Maneuvering through this maze can be maddening, and certainly risky.

There is obviously a great deal of fallacy out there. In general, most product claims remain unproven and many regimens require months and months (if not years) of use to appreciate a difference. Some people will want so badly to look and feel more youthful that they will swear something works, only to abandon it a short while later, after realizing things really have remained the same.

So what’s the good news? There really are methods that actually do increase life span. Though not “au courant”, calorie restriction has been confirmed over and over in studies, proving it increases longevity. This is disappointing news for some because it takes constant discipline. But as with most things, the work is worth the reward.

Life Span vs. Health Span

If the goal is to live longer while looking and feel younger, one has to consider that we are talking about a “health span” rather than a “life span”. After all, who wants to live to be nearly a century old if you are in pain, confined to a wheel chair, or (gasp!) look like you’ve passed away some years ago? As always, it’s much more about the life in your years rather than the years in your life.

It is understood that heavy-duty exercise will increase longevity and health, provided you don’t “overdo” it in the beginning, rather gradually build up your tolerance. The book series, “Younger Next Year”, demonstrates this case after case. Exercise is fully recommended by most practitioners. Additionally, there has been much attention given to the type II diabetes drug Metformin for helping every aspect of your body age slower, and possibly improve. This is because it inhibits the production of glucose in the liver and lowers insulin resistance, which in turn reduces the negative effects of glycation in the tissues – which is destructive over time.  Some experts recommend 500mg twice daily. The side effects of this drug are minimal in the presence of healthy kidneys. I am following the FDA’s clinical trials of the drug and it’s efficacy in regards to anti aging. I suggest you follow along as well on the government’s Clinical Trials site.

As always, my recommendation is to tread slowly and carefully when trying new things. Proven “anti-aging” regimens that I can recommend remain to be calorie restriction and exercise. These will never let you down. Also, smiling whenever possible does keep positivity at the forefront, which is necessary for a long, contented life.

Thread Lifting: A Lunch Time Facelift?

There’s a new (or newish) option for lifting and tightening sagging facial skin without going under the knife. Late last year, the FDA approved a new variation of a once-popular procedure, called a thread lift, making the use of temporary, absorbable sutures a viable option for those who want to achieve similar results of a traditional facelift without the surgery or downtime.

The original thread lift came out in the late ’90s and was performed up until the mid 2000s. The procedure fell out of favor about 10 years ago, but two recent options utilizing new technology, Nova Threads and the Silhouette Instalift, are bringing the thread lift back into the spotlight. While the sudden resurgence of the thread lift (sometimes referred to as a “lunchtime lift”) may be perplexing for some, doctors who are performing this noninvasive procedure say that the newer technology and the fact that it can be done in an hour are two big reasons that procedure is coming back into popularity as a facelift alternative.

How Does a Thread Lift Work?
A thread lift is exactly what it sounds like.  The sutures are inserted into the skin and can instantly tighten and lift problem areas with volume loss like the cheeks, jowls, lips and neck, while stimulating the body to build collagen in the area where the sutures are placed. Attached to the fine threads are tiny cones, small umbrella like attachments that lift and tighten from underneath the skin while simultaneously causing inflammation, which helps to stimulate collagen growth over time.

Another challenge was that the skin could only be pulled in one direction. “This made the results appear two-dimensional,” says Chicago plastic surgeon Julius Few, MD.  “The new threads are fully absorbable, making it an absorbable suture lift, not a thread lift. They can also be lifted in two directions, which results in a more three-dimensional look. The old procedure required overpulling of the skin to get a good result, making the downtime several days and not too different from what is now called a mini-facelift.”

Thread Lift vs. Fillers
So, how does the new and improved thread lift hold up against the popular “liquid facelift?” A thread lift procedure takes as little time as a treatment using a combination of fillers (about an hour) and recovery time is just one or two days. Results will vary, but can last anywhere from 18 months to a few years and additional threads can be used at a later time to maintain results. Because the cones continue to stimulate collagen, the results continue to improve over time. But, thread lifts won’t be replacing dermal fillers anytime soon. According to Dr. Few, to achieve optimal results its best to combine procedures. “Used alone, results can last up to 18 months,” says Dr. Few. “But we often use the absorbable suture lift in combination with fillers, lasers and other nonsurgical facial treatments to create a ‘stackable treatment,’” he says. “The results are expected to last several years when combined with other treatments and wear off gradually.”

Bye Bye Crepey Skin

Skin becomes crepey with age for a variety of reasons: buildup of extensive UV damage, dryness, aging, and we lose that precious but unappreciated (in youth) tiny layer of fat under the skin that keeps everything firm and smooth. The result is withering and puckering of the skin that takes on the effect of a continually deflating balloon.
There are many topical products that can help this situation…but only a little. Retinoids are the mainstay for keeping skin younger and healthy from the top down. New cells are produced faster and the entire skin is healthier. Problem is that it does not replace that little layer of fat that is forever gone. Even if one were to gain weight (don’t consider trying this at home) the fat would not return to that spot, just to other inconvenient areas.
One topical system can help however. Brand new research has determined that it is possible to wake up the stem cells congregated in the hair follicles. This produces new skin rather than rejuvenating older skin therefore there is no past damage to surface later. The new skin is young and healthy. There are 3 steps to this system called DEFENAGE®. This still does not replace the fat layer however.
There is an answer to this dilemma…placing a collagen stimulant under the skin to grow brand new collagen that is healthy, firm, and tightly wound will make the skin appear much like it did many years ago. Sculptra® is such a collagen stimulant, in fact the only one on the market. While it does have to be injected, the injections are nearly painless and can be done in as little as five minutes. You start to see new growth in approximately four weeks. The skin is stimulated to grow for well over 6 months but the majority of growth is seen in 3 months. A second treatment may be needed but nothing will bring back the beauty of youth like the new skin seen with Sculptra®.

Your Skin In A New Light with Laser Genesis

Want Palm Beach perfect skin?  Enter spring 2017 looking younger and feeling more confident with Laser Genesis.

What is Laser Genesis and what does it do?

It stimulates new skin cell growth and collagen production which smooths fine lines; shrinks pore size; evens out rough skin texture; and fades scarring.  This amazing laser procedure also closes down the tiny veins and capillaries that cause persistent redness of the skin.  Laser Genesis penetrates deep into the cellular layer of the skin. Chemical peels help to refine the surface while the laser corrects the problem at the cellular level. It is a non-invasive laser therapy that uses laser technology to gently heat the different levels of the dermis.

Laser genesis can be performed on several areas of the body, including the face, chest and neck. Many patients have actually described Laser Genesis as a pleasant treatment. The laser produces a warm, tingling sensation on the patient’s skin, which usually lasts for 30 minutes. It is effective for both men and women and is proven to increase an individual’s collagen production, which along with the heat generated by the treatment helps to shrink facial pores across the nose, cheeks, chin and forehead.

The 1064nm laser is color blind so it can be used on all skin types. The frequency is attracted to hemoglobin and water. Therefore, it will cause a renewal of collagen while shrinking the diffuse dilated blood vessels that contribute to the appearance of redness or Rosacea. Because the depth of penetration goes just beyond the pore depth, there is a tightening of the present pores resulting in a refined and refreshed appearance.

Laser Genesis can be used to treat many different issues and is a quick procedure to improve:

  • Facial Redness
  • Fine Lines & Wrinkles
  • Enlarged Pores
  • Rough Skin Texture
  • Hyperpigmentation & melasma
  • Acne scarring
  • Scars from trauma, surgery or burns

What can I expect after my Laser Genesis procedure?

The most common side effect is beautiful skin! You may experience a pinkness of the skin that can last up to 2 hours post procedure. Walk out with zero downtime and a healthy glow. Most people need a series of at least 6 treatments for the best results. Longevity of results are varied from person to person depending on their lifestyle, sun exposure, diet, and use of proper skin care products. Most people see their results lasting up to 12 months.

SculpSure™ + Sculptra® = Body Perfection

The past few years have seen some pretty phenomenal changes in regards to redefining your body without surgery. As new uses develop for older products and new machines actually do reinvent a better wheel, perfection is within our grasp!
We all know diet and exercise are mandatory not just for a fit body but for a solid state of well being. However that only works on appearance so long…eventually there are pockets of fat that nothing seems to relieve and the skin, well, it just isn’t the same. Cellulite begins to consume the backside, the knees find wrinkles from who knows where, and everything begins to sag: first a little, then a lot.
The first truly effective fat removing device (permanently removing) was Coolsculpt™. This device cooled a chunk of fat down to a temperature that was incompatible with the living cell, thereby destroying it. The only problem is that not only was the treatment dreadfully long, but it removed fat in various areas leaving a “shelf” of fat surrounding the target area. If you did not like that shelf (I understand no one did) then you had to undergo another treatment targeted to that particular area. This proved costly for patients expecting a more even result.
The latest effective fat removing device does not grab the fat and suck it into a container to treat it but rather uses 4 flat pads that transmit laser energy that passes through the skin and targets the fat layer at a 1.5cm depth. The skin is kept cool by circulating refrigerated water at the surface of the pads. Because there is a 30% radius of relayed energy, the fat is removed in a smooth, even fashion. The reduction is a bit more than is seen with the Coolsculpt device.
So now, the only thing that remains is the sagging, crepey, dimpling skin. Sculptra® is an injectable collagen stimulant, not a filler. When placed superficially under the skin, your skin is stimulated to grow fresh, healthy, new, stronger skin that will once again take you back to years ago. Placed in strategic areas of concern, you will once again relive those youthful years when the word “crepe” was reserved only for a paper you put out for birthday parties and baby showers.

Refyne & Defyne With Restylane

Introducing the Latest in the Ever-Growing Family of Restylane…

This past weekend I left my beautiful West Plam Beach for Dallas. I spent my time at the Galderma Headquarters getting to know the newest members of the Restylane family; Restylane Refyne® and Restylane Defyne®. While the Restylane family has been proven to be a fabulous dermal filler for decades, the addition of these products will not go unnoticed. Similar but with different crosslinking these two new gels are cohesive and extremely elastic, allowing for a continued smoothness even upon extreme facial animation. Think of a structure supported by a smooth but strong rubber band that stretches nicely but maintains it smoothness completely. Then it pulls back to its original soft form while obliterating lines and wrinkles. Its unique crosslinking leaves the product soft, smooth, and undetectable.
As we move along in life we lose the fine layers of fat directly beneath the skin. This fat covers a multitude of sins and makes the skin and face look flawless. As it dissipates the skin lies closer to the underlying muscles responsible for movement. That muscle begins to adhere to the skin making it resemble the muscle underneath, which is not terribly pretty. Restylane Refyne® or Defyne® can mimic the wonderful fat we used to have, once again making us look flawless!

The Beauty of Volbella

The Beauty of Volbella
The Juvéderm (Allergan) family has welcomed its latest addition: Volbella®. Volbella is a hyaluronic acid (like Juvéderm) but is manufactured to be a soft and subtle addition to the line of products that addresses lips, fine lines, and subtle issues that require a gentle enhancement.
The look of Volbella is refined especially around the mouth where we tend to accumulate the dreaded “smoker’s lines”. The area does not get elevated so the platypus look is avoided and swelling is nearly non-existent. The lips are gently enhanced into a perfect pout, rather than a protruding trout. The luxurious feel of this new filler is heartily welcomed by all!

The Loveliest Liquid Face Lift

The thought of receiving face-lift results without a scalpel is certainly compelling. Depending upon the patient, a variety of products can be used to provide face-lift results in 30 minutes or less!
This patient received lifting in the cheek area (cheeks should “lift” rather than protrude) with Juvéderm Voluma®. Voluma® is a hyaluronic acid that has a unique ability to lift and also resist degradation by skin and pressure to maintain a lift of your natural skin and volume. Dysport® is a neurotoxin (similar to BOTOX) that will soften the lines of the frown and crow’s feet. Juvéderm Ultra is a finer hyaluronic acid that will gently plump the lips without making them appear like a trout pout. Finally, the marionette lines (lines heading toward the chin on either side of the mouth) were plumped and softened by Restylane Lyft®, a robust hyaluronic acid that gently removes the furrows and creases. The goal of the Liquid Face Lift is to be “Fresh and Rested”, not “Plumped and Pouty”.

How to Keep up With the Flying Time

Have you ever thought about something that you think you just did maybe two to three weeks ago and then find out that it was several months ago? In my case I will see a patient on my schedule and think “wow, I just saw them” and when I check the chart it turns out that it was over 6 months ago! What’s happening?
We are bombarded moment by moment by local events, information, schedules, family, and ever frightening world events. How we even manage to complete a day without disaster sometimes seems amazing.
Keeping Balanced
One of the most important things to do is to plan some non-negotiable time for reflection, meditation, Bible reading, or whatever grounds you. Without this time your day is reactionary rather than planned. Walking alone is a wonderful means to clear your head and prepare yourself for the upcoming assaults of the day. Making lists (and remembering to look at them) can also help to keep a sense of organization going, which can be calming throughout the day.
Because time is flying by so fast, we often forget about ourselves. In my world, I remind my patients to make sure that they stay on their skin care and have enough of the product on hand to not have to worry about running out. Since I only recommend evidence-based skin care, the products my patients use are actually keeping their skin not only beautiful but healthy. Apply it and forget it. In addition, keeping on a schedule to receive the little maintenance treatments such as BOTOX helps to prevent that feeling we get when we look in the mirror as if for the first time and say “what the heck happened?!”  Also remember, you’re not alone. The time is flying by for everyone. You can even ask your kids!
